To set up your complementary consultation:
Call our office at (916) 631-8046 we will help you schedule a convenient
appointment time.
We use a wonderful, comprehensive health history form to learn about you
and your health concerns. To save us both time, we want you to bring it in
already filled out from the comfort of your home. We can either mail it to
you, or you may print it out right HERE, and bring it with you. (You will need
Acrobat Reader to view and print the questionnaire. If you do not have
Acrobat Reader, you may download it at no charge HERE.)
Also, many people choose to come to a Health Talk. These informative &
entertaining programs are given, at no charge, 2 times each month. Please
take a look at the Events Calendar for the next date and time.
